PLEASE NOTE THAT ALL CHANGE OF DETAIL REQUESTS CAN TAKE UP TO 5 WORKING DAYS. YOU WILL BE NOTIFIED VIA TEXT OR EMAIL ONCE YOUR DETAILS HAVE BEEN UPDATED.

    Proof of address change may need to be provided. If proof is required you will be contacted to supply this via email or phone.

    It may take up to 5 working days for address changes to be updated on our system, once this has been completed you will be notified of the change via text. 

    If your new address falls outside of our practice catchment area, you will be sent an out of area letter. The out of area letter will advise you to re-register with the closest GP practice to your new home address.

    I understand that this form collects my name, date of birth and any other personal information that I choose to provide. This is to confirm I am registered with the practice ; to allow the practice team to contact me and to update the medical records held by them and their partners in the NHS. I understand that I am able to read the privacy policy (located in the footer of the website) to discover how they protect and manage my submitted data.
